The cover didn't fit
Our first cover prototype came back with tolerances too tight to seat properly on the housing. Rather than force it, we went back and redesigned the cover geometry from scratch, adjusting the lip depth.

The adhesive used to seal the interior (critical for waterproofing) refused to dry properly under factory conditions. We tested multiple compounds before finding one that bonded correctly and held under thermal cycling.
